Let's Begin!
Open up a new image I like to use a tagger size of 461 x 461
Flood fill white
Then using your ellipse tool and your width setting set at 8, and using this color #8da1a8 and making sure your foreground and stroke is open and your background and fill closed, draw out a circle. Convert to raster, then go to Adjust-Add/Remove Noise- Add Noise- Gaussian and monochrome checked and noise at 100%. Then go to Selections-Select All-Float-Defloat-Modify-Expand by 4 then open up paper 4 and right click copy and paste as new layer under your circle frame, then go to Selections-Invert-Delete-Deselect
Open up your tube and right click and copy and paste as new layer place her below your circle frame then duplicate your tube and place the duplicate above the circle frame. Close your duplicate layer. On your circle frame using your magic wand click inside the circle frame then go to Selections-Select All-Float-Defloat-Modify-Expand by 4 and then on your original tube layer go to Selections-Invert-Delete-Deselect. Open up your duplicate tube and erase the parts of the tube sticking out of the circle.
Open up the doodle element and right click copy and paste as new layer and place below the circle frame, duplicate and mirror, then duplicate again and go to Image-flip, then on the first duplicate you mirrored duplicate that one and then go to Image-flip. On all of the doodle go to Adjust-Add/Remove Noise-Add Noise- Gaussian and monochrome checked and noise set at 100%
Open up the poinsettia element and right click copy and paste as new layer and duplicate 5 times giving you a total of 6 and place how I have mine. Open up the candy cane and copy and paste as new layer and place where I have mine.
Open up paper  3 and copy and paste as new layer and add your mask, invert your mask
